LibQUAL+ @ Lewis Library
WHAT
Lewis Library is using the LibQUAL+ survey to measure library user's perceptions of the quality of our services and to plan for future service improvements. The survey takes 10 to 15 minutes to complete.

WHY
Lewis Library cares about your opinions and is gathering feedback in order to identify areas of strength and weakness. Information gathered in the survey will help the library to improve services. |
WHO
UNTHSC students, staff, faculty, and all other users of Lewis Library are invited to participate. Completing the survey is voluntary.

WHEN
The LibQUAL+ survey will be available from April 10 through May 10. |
WHERE
At your desktop or on your laptop computer! You will receive an email message with a link to the survey.

WIN
If you choose to enter our drawing, you could win one of five exciting prizes: an iPod nano, iPod shuffle, or $25 to $50 in credit for library printing and copying.

CONFIDENTIALITY - We take your confidentiality seriously and have taken the following steps to protect your interests.
- Name not required or requested for completion of LibQUAL+ survey
- Provide email address only if you want to enter prize drawing
- Individual survey responses are housed on the servers of a private hosting facility and not shown to Lewis Library staff
- Data from responses is reported back to Lewis Library in an aggregated format only
- Email addresses provided as entries for the prize drawing are kept servers different than survey responses
- Comments made on the survey are reported back to Lewis Library, but only anonymously
- LibQUAL+'s parent organization, the Association of Research Libraries, follows the Ethical Principles of Psychologists and Code of Conduct of the American Psychological Association with regard to confidentiality and privacy
